Doctor’s name and office information
-
Patient name
-
Prescription issue and/or expiration date
-
Sphere, cylinder, axis and add power (if applicable)
-
Prism information (if applicable)
Certain prescriptions and lens types may require additional measurements including pupillary distance (PD), segment heights or fitting measurements.

JSpecs Opticians reserves the right to verify prescription information with the prescribing doctor before processing any order. Orders may be delayed if:
-
Prescription information is incomplete
-
Prescription details are illegible
-
Required measurements are missing
-
Verification with the prescribing provider is necessary
For patient safety and optical accuracy, JSpecs Opticians may decline orders that cannot be reasonably verified.
Prescription Expiration
Customers are responsible for ensuring their prescription is current and valid prior to ordering. Expired prescriptions may not be accepted.
JSpecs Opticians reserves the right to refuse fulfillment of expired prescriptions or prescriptions that do not meet applicable regulations or standards.
Pupillary Distance (PD) Measurements
Customers submitting their own PD measurements acknowledge that inaccurate measurements may affect visual performance, comfort and lens accuracy.
JSpecs Opticians is not responsible for issues resulting from self-measured or incorrectly submitted PD information.
Lens Accuracy & Industry Standards
All prescription lenses are manufactured within accepted ANSI optical tolerance standards. Minor tolerances may exist within industry-approved manufacturing guidelines.
Certain prescriptions, lens materials and frame selections may result in:
-
Peripheral distortion
-
Thickness variations
-
Cosmetic asymmetry
-
Reduced edge clarity
-
Adaptation periods
Progressive and multifocal lenses may require an adjustment period as the wearer adapts to new viewing zones and lens designs.
Customer Responsibility
Customers are responsible for reviewing and confirming:
-
Prescription information
-
Frame selection
-
Lens options
-
Shipping information
-
Measurements submitted at checkout
Orders placed with incorrect prescription information or customer-submitted measurement errors may not qualify for refunds or remakes.

Customer-Owned / NOS / Vintage / Refurbished Frame Policy & Waiver
Customer-Owned Frames
JSpecs Opticians is happy to work with select customer-owned frames when possible. However, customers acknowledge that previously worn, vintage, discontinued, NOS (new old stock) or refurbished frames may carry increased risk during the lens installation and adjustment process.
Older acetate, plastic and metal materials may become brittle or weakened over time, even if no visible defects are present.
No Guarantee Against Breakage
While every reasonable precaution is taken during handling and fabrication, JSpecs Opticians cannot guarantee against breakage, stress fractures, finish damage or component failure when working with:
-
Customer-owned frames
-
Vintage frames
-
NOS frames
-
Discontinued frames
-
Refurbished frames
-
Previously adjusted or repaired frames
By proceeding with lens installation, the customer acknowledges and accepts these risks.
Limited Warranty on Customer-Owned Frames
JSpecs Opticians does not provide warranty coverage on customer-owned, vintage, NOS or refurbished frames unless otherwise stated in writing.
Warranty coverage applies only to newly fabricated prescription lenses and only within applicable manufacturer limitations.

Certain prescriptions, lens materials and frame designs may not be compatible. JSpecs Opticians reserves the right to decline work on frames deemed unsuitable for:
-
High prescriptions
-
Drill mount applications
-
Rimless conversions
-
High-wrap designs
-
Structural instability

Unsafe mounting conditions
Recommendations may be made for alternative frame selections to improve safety, durability and optical performance.
